Design: Dialog synch to Gitlab

I need a design for a modal dialog.

It should be used to start the synch process from the InformationPortal towards GitLab. In this dialog the user is asked to define the repo and the branch. However, this information is not mandatory. Furthermore, the user can add a commit message (not mandatory).
