approval workflow support

fix #68 (closed) (also documented in README) additional changes:

  • refactoring of CONFIG use and validation
  • dynamic module loading (used with backend, notifier, db) -> fix #67 (closed)
  • increase test coverage (User, ldap, unix, db, name generation, ...) -> address #69 (closed) , fix #11 (closed)
